Payment Plan Option and Fees
Fees for home birth service range from $6,000-$7,500 depending on the payment plan chosen by the client, when the client enters care and whether a distance fee is included. If applicable, the $75 consultation fee will count toward the home birth service cost at contract signing.

-
Please see payment plan table below for payment options.
-
A $500 distance fee will be assessed if care and birth will be provided more than 25 miles from the Scottsdale Road /101 Freeway location.
-
A $500 non-refundable fee is due at contract signing (no later than the 2nd visit) for all clients and will count towards the overall global fee.
Subsequent payments are typically due at prenatal visits. All payments can be paid by cash, check or bank-issued money order. A superbill can be provided after delivery in hopes of insurance reimbursement. Payment plan timelines can be altered if the client enters care after 16 weeks. Once the Midwife-Client Contract is signed, the client will be considered a confirmed patient of Living Hope Midwifery PLLC and the client's eventual due date will be reserved on the calendar.
